Service Agreement & Data Authorisation

This Service Agreement and Data Authorisation (the "Agreement") is entered into by and between:

Tianjin Centre for International Enterprise & Culture Exchange ("TIECEC", "we", "us", "our"), a registered organisation located at Qihang Building, Anshan West Road, Nankai District, Tianjin, People's Republic of China, which operates the TIECEC Global Care Plan ("TGCP"); and

You (the "Client", "you", "your"), the individual who accepts this Agreement by clicking "I agree and sign" at the end of this page.

By clicking "I agree and sign", you acknowledge that you have read, understood and agree to be bound by this Agreement.

1. Definitions

• Coordination Services: the non-medical coordination services described in Section 2.
• Medical Provider: the hospital(s) and physician(s) that provide diagnosis and treatment to you.
• Travel Agency: the licensed travel agency that arranges accommodation, transport and excursions.
• Medical Records: your medical history, diagnostic images, reports and related health information.

2. Scope of Services

TIECEC, through TGCP, provides the following coordination services only:

1. Pre-consultation — gathering your medical history and preparing you for assessment;
2. Facility matching — identifying and recommending hospitals and physicians that fit your needs;
3. Itinerary design — planning your travel, stay and treatment schedule;
4. Bilingual companion — a bilingual coordinator accompanies you throughout your journey;
5. Settlement — assistance with billing and payment coordination;
6. Follow-up — remote follow-up after you return.

3. Nature of Services — Not Medical Care

TGCP is a coordination and support service. We are not a hospital and we do not provide medical diagnosis, treatment or advice. All medical decisions are made by you together with your treating physician. Any statement about a hospital or physician is for information only and does not constitute a medical recommendation.

4. Fees and Payment

• Service fee: a flat fee of US$2,000, covering the coordination services in Section 2.
• Introductory voucher: the first 100 clients receive a US$500 voucher, reducing the fee to US$1,500.
• No hidden escalation: the service fee is fixed and will not increase if your journey is extended or your condition changes.
• Timing: the service fee is paid in full when you accept this Agreement.
• Method: payment is made by international bank transfer to TIECEC's corporate account in mainland China. Medical fees are paid directly to the hospital, and travel costs to the licensed travel agency, after you arrive in China.

5. Your Obligations

You agree to:

1. provide true, accurate and complete Medical Records and personal information;
2. disclose any conditions or circumstances that may affect the safety or feasibility of your journey;
3. cooperate reasonably with TIECEC, the Medical Provider and the Travel Agency;
4. comply with applicable laws and the rules of the hospital and travel agency.

6. Third-Party Services

Hospitals, physicians and travel services are provided by independent third parties, not by TIECEC. Each third party invoices you directly for its own services. TIECEC is not responsible for the acts or omissions of third parties, and any claim arising from third-party services must be made against the relevant third party.

7. Data Authorisation and Privacy

You authorise TIECEC and the Medical Provider to collect, process, store and transmit your Medical Records and personal information, solely for the purposes of providing the coordination services and facilitating your medical care.

• This authorisation is limited to what is necessary for those purposes.
• You may withdraw this authorisation at any time by written notice to tgcp@tiecec.com. Withdrawal does not affect processing that has already occurred.
• Your information is handled in accordance with our Privacy Notice.

8. Confidentiality

TIECEC and the Medical Provider will keep your Medical Records and personal information confidential, and will not disclose them except as required by law or as necessary to provide the services.

9. Informed Consent (signed at the hospital)

Before any treatment, your treating physician will explain the treatment plan, its purpose, alternatives and risks, and you will sign a separate informed consent at the hospital. That consent governs the medical treatment itself.

10. Limitation of Liability

To the fullest extent permitted by law, TIECEC's total liability under or in connection with this Agreement is limited to the service fee you have paid. TIECEC is not liable for any medical outcome, or for any loss arising from the acts or omissions of the Medical Provider or the Travel Agency.

11. Force Majeure

Neither party is liable for delay or failure caused by events beyond its reasonable control, including natural disasters, epidemics, government actions and similar events.

12. Term and Termination

This Agreement takes effect when you accept it and continues until the coordination services are completed. Either party may terminate this Agreement by written notice. If you terminate before the services begin, TIECEC will refund the service fee, less any costs already incurred.

13. Governing Law and Dispute Resolution

This Agreement is governed by the laws of the People's Republic of China. The parties will first attempt to resolve any dispute through friendly negotiation. If negotiation fails, the dispute will be submitted to a court of competent jurisdiction in Tianjin, People's Republic of China.

14. Notices

Notices under this Agreement will be sent to tgcp@tiecec.com (for TIECEC) or to the contact details you provided (for you).

15. Entire Agreement and Severability

This Agreement is the entire agreement between the parties concerning its subject matter. If any provision is found invalid or unenforceable, the remaining provisions remain in full force.

16. Amendments

TIECEC may amend this Agreement. The version in force at the time you accept it applies to you.

17. Acceptance

By clicking "I agree and sign" below, you accept this Agreement and confirm that you are at least 18 years old and have the legal capacity to enter into this Agreement.
